You may complete the evaluation by (1) using AbleNet’s online evaluation tool, (2) uploading your own completed evaluation, or (3) using specific forms as required by the insurance company/state.

When completing data sheets, if possible, try to include a variety of pragmatic functions your client used the SGD for (requesting, commenting, social, giving/receiving information, etc.)

great examples to include on your data sheets or within your evaluation if data sheets are not required.
